USE CASE: Creating a New Ambulatory Care Model

This healthcare provider was awarded a cooperative agreement from the Centers for Medicare & Medicaid and Medicaid Innovations to develop intervention programs to improve health and reduce costs for Medicaid, Medicare, and Child Health Plan (CHP) populations.

Project Summary: To assist in the creation of a 21st century ambulatory health care model with scalable and replicable solutions to improve current limitations.

Benefit to Colorado: A new ambulatory health care model will help providers across the state lower costs and improve care.
