USE CASE: Understanding Bundled Payment Opportunities

A nonprofit health plan that provides access to behavioral and physical health services for Coloradans.

Project Summary: This health plan was a part of a bundled payment learning collaborative aiming to analyze and understand bundled payment opportunities. They hope to use CO APCD data to provide personalized support to collaborative members and prepare them to move forward with bundled payments.

Benefit to Colorado: This request helped the Health Plan develop expertise on this type of payment model and provided information on possible medical episodes for which a bundled payment system would be most likely to reduce/eliminate potentially avoidable complications (PACs). Bundled payments are on the forefront of improving care and lowering costs.
