This healthcare provider was awarded a cooperative agreement from the Centers for Medicare & Medicaid and Medicaid Innovations to develop intervention programs to improve health and reduce costs for Medicaid, Medicare, and Child Health Plan (CHP) populations.
Project Summary: To assist in the creation of a 21st century ambulatory health care model with scalable and replicable solutions to improve current limitations.
