We have created a Spanish version of the front side of the form to facilitate discussion, but the official MOST form, signed by physician/APN/PA and patient or patient’s authorized decision maker MUST be the two-sided, English-language, official form. The Spanish version is not an actionable medical order; it is only to be used in the discussion of choices.
