Improving Access to Palliative Care Across Colorado

The Colorado Department of Public Health and Environment (CDPHE) leads a multi-stakeholder effort to assess and improve access to Palliative Care in Colorado. CIVHC worked collaboratively with CDPHE to create the interactive map below that patients and providers can use to locate Palliative Care Services near them. The map accompanies a larger report that also addresses key factors to help enhance statewide access to Palliative Care services. The data in the map and report is based on a survey performed by Hospice Analytics with financial support from CDPHE, Colorado Department of Health Care Policy and Finance (HCPF), and the American Cancer Society Cancer Action Network.
